International Football Association Board (IFAB) rules do state that 'if a player who is to be replaced refuses to leave, play continues.'
I didn't know the above. I would have thought if they refused to leave, the referee could force them by giving them a yellow card or even security getting involved in most extreme cases.
But I guess Kepa did....

City captain Vincent Kompany, meanwhile, said he had never seen anything like Kepa's refusal to go off, but was glad not to face Willy Caballero in the shootout.
'I know how good Willy is on penalties, when we won this competition against Liverpool he was tremendous, he won the penalties for us and it would be another mind game to be played,' he told Sky Sports.
'It didn't happen and the confidence was there to score the penalties. Of course I've never seen anything like it. I wish I could do it every now and then when I don't want to get subbed off!'

To be fair this wouldn't have happened if JT was captain or even with a Roy Keane, Vieira captain....

The full back is not actually club captain at Stamfod Bridge, with that responsibility bestowed on the ostracised Gary Cahill. Due to the former England defender's continued absence, Azpilicueta has worn the armband throughout the season.
However, his lack of leadership skills was hammered home ahead of the penalty shootout, when he had to ask Luiz which end he believed Chelsea should select after winning the toss.
