I think we southerners are t he biggest culprits. After pretty spending significant time in nearly every region of the country, I've learned that everyone has their own way of speaking, but I think the southeast is the only place in the country where people from the outside can go and not really understand what people are saying.
Like early in our relationship, my wife told me it was fascinating to hear me talk to people from back home because she couldn't always understand what I was saying and it was weird to her because when we talked, other than a hint of accent here and there, there was never any problem.
