from what i read, somewhere else, the dude they are blaming was either warned of the possible opportunity for a breach or it was he himself, who told the higher ups about it, and they took no action

More than likely the latter, its never a problem for higher ups until it's too late or they're CC'd on an urgent email. The corporate world is so disgusting. It's a bunch of dudes doing a job to get a better job with less work. At the same time never actually understanding or doing the job they currently have. Just climb, climb, climb, with no cause and no direction but self survival.
